Pharmacokinetics

Suction

The bioavailability of alendronate at a dose of 70 mg when taken orally on an empty stomach 2 hours before a standard breakfast was 0.64% in women and 0.59% in men. When taken 1 hour or 30 minutes before breakfast, the bioavailability of alendronate decreased to 0.46% and 0.39%, respectively. Clinical studies have confirmed the effectiveness of alendronate when administered at least 30 minutes before the first meal or drink. The bioavailability of alendronate is negligible when taken with food or within 2 hours after a meal. Distribution

Removal

Indications

Contraindications

- strictures or achalasia of the esophagus and other conditions leading to a slowdown in the movement of food through the esophagus;

- the patient’s inability to stand or sit for 30 minutes;

- hypocalcemia;

- severe renal failure (creatinine clearance less than 35 ml/min);

- severe disturbances of mineral metabolism;

- pregnancy;

- lactation period (breastfeeding);

- children's age (efficacy and safety of use have not been established);

- hypersensitivity to alendronate or other components of the drug.

Carefully: diseases of the gastrointestinal tract, such as dysphagia, gastritis, diseases of the esophagus, duodenitis, peptic ulcer disease in the acute phase, active gastrointestinal bleeding or surgery in the upper gastrointestinal tract during the year preceding the start of treatment; hypovitaminosis D.

Dosage

The optimal duration of use of the drug has not been established. The need for continued bisphosphonate therapy should be assessed regularly, especially after 5 or more years of use.

Side effects

Overdose

Symptoms: abdominal pain, dyspeptic disorders, dysphagia, heartburn, esophagitis, gastritis; Hypocalcemia and hypophosphatemia may develop.

Treatment symptomatic. The use of milk and antacids to bind alendronate is indicated. Due to the risk of damage to the esophagus, vomiting should not be induced and the patient should be in an upright position.

Drug interactions

Concomitant use of calcium supplements (including dietary supplements) and certain other drugs for
oral administration, as well as food and liquid intake (including mineral water) impairs the absorption of alendronic acid. In this regard, it is recommended to take other medications no earlier than 30 minutes after using Forosa. NSAIDs (including) may enhance the side effects of alendronic acid on the gastrointestinal mucosa.

In clinical studies in patients taking estrogen-containing products (intravaginally, transdermally, or orally) concomitantly with alendronate, no clinically significant interactions were observed.

Although specific drug interaction studies have not been conducted, the use of alendronate in clinical studies with a large number of commonly used drugs was not accompanied by the development of clinically significant interactions.

The decision to proceed with treatment must be made individually for each patient after a careful risk/benefit assessment, especially for patients with Barrett's esophagus.

When treating with alendronate, there are known cases of adverse reactions from the esophagus (esophagitis, ulcer or erosion of the esophagus), sometimes severe and requiring hospital treatment and in rare cases complicated
formation of a stricture.

It is necessary to monitor the possibility of any adverse reactions from the esophagus. The patient should be informed of the need to stop taking the drug and consult a doctor if dysphagia, pain when swallowing, chest pain and heartburn develop.

In post-marketing reports, rare cases of gastric and duodenal ulcers have been reported, sometimes
severe and complicated, although no increase in this risk was noted in extended clinical studies of alendronate.

The risk of severe esophageal side effects is greater in patients who take alendronic acid not as directed in this label and/or who continue to take it after symptoms suggestive of esophageal irritation have occurred. It is important to explain in detail to the patient the rules for taking the drug and make sure that he understands them. Patients should be aware of the increased risk of adverse events from the esophagus if they deviate from the instructions.

Before starting therapy with Forosa, correction of hypocalcemia and other metabolic disorders (such as vitamin D deficiency) is necessary. Due to the increase in bone mineral density during alendronic acid therapy, a slight clinically asymptomatic decrease in serum calcium and phosphate concentrations is possible, especially in patients receiving corticosteroids, in whom calcium absorption may be reduced. Therefore, it is important to ensure that sufficient amounts of calcium and vitamin D enter the body, which is especially important in patients receiving corticosteroids.

Patients should be warned that if they accidentally miss a dose of the drug according to the once-weekly regimen, they should take 1 tablet in the morning of the next day (it is unacceptable to take 2 tablets in one day). In the future, you should continue to take 1 tablet on the day of the week that was chosen at the beginning of therapy.

There is also information about the development of osteonecrosis of the jaw in patients with osteoporosis receiving oral bisphosphonates. When assessing an individual's risk of developing osteonecrosis of the jaw, risk factors such as:
bisphosphonate activity (highest for zoledronic acid), route of administration and total dose of the drug; oncological diseases, chemotherapy, radiation therapy, taking corticosteroids, taking angiogenesis inhibitors, smoking; history of dental disease, poor oral hygiene, gum disease, invasive dental procedures, poorly fitting dentures. Before starting oral bisphosphonate therapy, patients should undergo a dental examination with appropriate preventive dental treatment. During treatment, these patients should avoid invasive dental procedures if possible. For patients who develop osteonecrosis of the jaw during bisphosphonate treatment, dental surgery may worsen the condition.

There are no data regarding the possible reduction in the risk of osteonecrosis of the jaw after discontinuation of bisphosphonates in patients requiring dental intervention. In every case
the decision should be made by the attending physician based on an assessment of the ratio of the expected benefit to the possible risk for a particular patient. The importance of proper hygiene should be taught to patients during bisphosphonate therapy.

Bone, joint and/or muscle pain has been reported in patients receiving bisphosphonates. These symptoms were rarely severe and/or disabling. The time of onset of symptoms varied from one day to several months from the start of therapy. In most patients, symptoms resolved after cessation of treatment. In some patients, symptoms reappeared when the same drug or another bisphosphonate was resumed.

Pathologic (i.e., low-force or spontaneous) subtrochanteric fractures and proximal femoral shaft fractures may occur in patients taking alendronic acid long-term. Fractures can occur after minimal or no trauma. Some patients may experience hip pain, often with outward signs of stress fractures weeks/months before a complete femur fracture occurs.

Atypical fractures of the proximal femoral shaft were often bilateral, so patients

But now Foros is a quiet, calm, somewhat provincial place, where the Soviet Union still breathes. There are no parties, noisy clubs, places of tourist pilgrimage or other social life to be found there. Even climbers do not settle in it, although the Forossky Kant rock massif, which is located much higher than the village, is for climbers.

In general, you should go there only if you want to take a break from crowds of people, noise and the frantic rhythm of the city. In those moments when you just need to go to the depths of the world and not see or hear anyone, and live a quiet, simple life with minimal signs of civilization.

When I arrived there, I was a little taken aback by the stagnation, emptiness, lack of signs of vigorous activity and silence. I was just in a non-Foros state at the time. But in moments when I wanted to be alone with myself and my thoughts, with full confidence that no one and nothing would distract me, I, dripping with saliva, remembered Foros and dreamed of being there. Away from the noisy tourist crowds and bustle. In general, this place is for a certain state of mind.

How to get there

Foros is located on the Crimean Peninsula, which can be reached by all means known to us: on foot, by bicycle, by boat and even on horseback. And of course, by car, bus, train and plane.

By plane

There is only one civil airport in Crimea, in Simferopol - SIMFEROPOL International Airport. There is a military airfield in Sevastopol, but planes full of tourists, unfortunately, do not fly there. But you can fly to almost all leading airlines. There are on average 5–7 flights a day from Moscow, St. Petersburg, Yekaterinburg and other cities with a population of over a million, and if you take care of tickets in advance, they will be quite cheap: about 7,000 rubles. Path from the airport

Taxi

As at every airport, you will be greeted here by friendly but greedy taxi drivers. A trip from Simferopol to Foros, including luggage, will cost around 3,000 rubles. The journey will take about an hour.

Bus/trolleybus

For fans of public transport, there are several options for the development of events. In any case, you need to go with a transfer in Yalta.

If you want to take a leisurely ride along the South Coast Highway, stopping at every stop and enjoying the mountain and sea views, then you can use trolleybus No. 55, following the route: airport -. These trolleybuses run rarely, but on any other city trolley you can get from the airport to the railway station, and from where trolleybuses No. 52 and other buses go towards Yalta every 20 minutes. The bus station, from where trolleybuses and minibuses depart, is located opposite the main entrance to the airport, across the parking lot, immediately behind the fences.

Minibus No. 128 runs from Yalta to Foros every 40 minutes. The last one seems to be around 8-9 o'clock. It departs from the upper platform of the bus station.

There is another option to take any Sevastopol bus from Yalta (they run every 20 minutes) and get off at the Foros stop on the highway. But then you will have to walk 3 kilometers down from the highway. With suitcases this is not the most pleasant experience.

By train

For lovers of the romance of trains, Russian Railways came up with a “Single Ticket” to Crimea. By train from all major cities of the country you can get either to Anapa or Krasnodar, from there by bus to the ferry crossing. Then you find yourself in Kerch and there you take other buses to travel to the villages and villages of the Crimean Peninsula.

But personally, I don’t see the point in such a ticket: making a bunch of transfers in the heat, and even the risk of being stuck at the crossing for several hours is very high. The tourists traveling on the “Single Ticket”, whom I met at the stations, had a very sad and “rumpled” look, they were no longer happy about coming to Crimea, fatigue from numerous road scrapes took over.

This option may be relevant for residents of remote regions, where flight fares are quite high, in order to save money. Getting to Crimea by plane from Central Russia is not at all expensive.

By bus

There are regular buses from Moscow and St. Petersburg to Kerch. The ticket price is about 3 thousand rubles. Having reached Kerch, you will need to get to the city you need on your own. Fortunately, the bus service between Kerch and the rest of Crimea is quite well developed.

Check-in time at the Foros sanatorium is 14 hours, check-out time is up to 12 hours, while the last service is breakfast.

The sanatorium allows pets up to 5 kg. Payment 600 rubles per day to the sanatorium cash desk.

If the Guest checks into the room after 14:00 of the current day local time, payment for accommodation will be charged in the same amount if check-in was made at 14:00.

There is no hourly payment for accommodation in the Sanatorium.

When placing before check-out time and if there are rooms available and ready for occupancy, a fee will be charged in the following order:

 100% of the room rate upon arrival before 00:00;

 50% of the room rate upon arrival from 00:00 to 12:00 hours.

Upon expiration of the paid period of stay, the Guest is obliged to vacate the room no later than 12:00 or extend the period of stay in the room. In case of untimely vacancy of the room by the Guest, the Administration has the right to evict the Guest without his participation.

In case of extension of the Guest's stay and subject to availability of rooms, fees will be charged in the following order:

 50% of the room rate upon check-out before 18:00;

 100% of the room rate when checking out after 18:00.

To check into the Foros sanatorium, you must contact the accommodation service and provide a passport and voucher, and in case of undergoing medical procedures, you must additionally provide a sanatorium card. When staying with children under 14 years of age, you must provide a birth certificate for the children.

Treatment at the Foros sanatorium

In the Foros sanatorium you can not only relax and enjoy your vacation on the Black Sea, but also undergo a course of treatment. Children are provided with medical or health services included in the price of their stay from the age of 5 years. Medical services are not provided to children under 5 years of age.

Sanatorium-resort treatment is carried out in the presence of:

 for children under 18 years of age – vaccination certificates and certificates of the epidemiological environment (issued at the place of residence);

 for adults over 18 years of age - a sanatorium-resort card - form No. 072/U-04 (issued at the place of residence).

A health resort card is not issued at the sanatorium!
